Plain version: if you fill out a form here, I get your name and email so I can send you what you asked for and stay in touch. I use standard advertising and website tools that count visits. I do not sell your information to anyone, ever.
This policy is about information collected through drivewellpt.com: the forms, the booking calendar, the chat widget, and the ordinary tracking that comes with running a website and advertising on it.
It is not my Notice of Privacy Practices for patients. Protected health information created in the course of care is handled separately, under HIPAA and Nebraska law, and is governed by the notice you receive as a patient. If you are an existing client asking about your clinical records, that notice applies, not this page.
Information you give me. Your name, email address, and phone number when you fill in a form, request the free guide, book a discovery call, or message me through the chat widget. Anything else you choose to type into those boxes.
Please do not send detailed medical information through the website forms or chat. They are not the right place for it. We will cover your history properly once we are working together, in a setting built for it.
Information collected automatically. Standard web and advertising data: your IP address, browser and device type, which pages you viewed, how you arrived here, and whether you came from an ad. This is collected through cookies and similar technology.
This site runs the Meta pixel, a small piece of code from Meta (Facebook and Instagram). It tells me how many people visited after seeing an ad, and lets me show ads to people who have visited the site. It does not tell me who you are by name, and I do not send Meta any information about your health.
You can limit this. Meta's ad settings let you control how your activity is used, at facebook.com/adpreferences. Your browser settings can block or clear cookies. Most browsers also offer a Do Not Track setting, though there is still no agreed standard for how sites should respond to it, and this one does not.
I use a small number of service providers to run the business. They process information on my behalf and are not permitted to use it for their own purposes.
I do not sell your personal information, and I do not share it with anyone for their own marketing. I may disclose information if the law requires it, or to protect someone's safety.
If you opt in, I will send occasional emails. Every one has an unsubscribe link at the bottom and it works immediately.
If you give permission for text messages, you can stop them by replying STOP to any message. Message and data rates may apply. I will not share your phone number with anyone for their marketing.
Contact information stays in my CRM until you ask me to remove it, or until it has clearly gone cold. Ask and it goes.
You can ask me what information I hold about you, ask me to correct it, or ask me to delete it. Email troy@drivewellpt.com and I will take care of it. Depending on where you live you may have additional rights under your state's privacy law, and I will honor those.
This website is meant for adults. I do not knowingly collect information from anyone under 13 through it. If you believe a child has submitted information here, email me and I will delete it.
The site runs over an encrypted connection and my providers maintain their own safeguards. No system online is perfectly secure, and I am not going to pretend otherwise, but your information is handled carefully and shared with as few parties as possible.
If this changes in a way that matters, I will update the date at the top of this page.
